We have an excellent opportunity for a full-time advanced practice primary care provider (APC-PCP) to join our expanding Geriatrics program. Join a collaborative team that provides comprehensive primary care and advanced care planning for a specific panel of geriatric patients in the home setting including patients’ homes, Assisted and Independent living facilities. Through a whole-person and innovative approach to patient care, our Geriatric providers deliver personalized care. Our providers talk with, listen to, and develop strategies with our patients and their families in the comfort of their own homes to keep them healthy.

  • Opportunity to work autonomously and also be a member of a cohesive team
  • Focused geriatric in-home care for frail and at-risk patients with multi-morbidity who cannot travel to the office setting
  • Collaborative and innovative practice environment with team support embedded in our medical home model (RN triage, MA support for referrals & refills)
  • Small panel, spend more time with each patient, care for 6-8 patients daily
  • Full-time with shared call responsibility

Required Qualifications:

  • Nurse Practitioner (NP) – Must be nationally certified as an FNP or AGNP, graduate from an accredited master’s degree NP program and possess state licensure to practice as an APRN. Current license to practice professional nursing in the Commonwealth of Massachusetts
  • Physician Assistant (PA) – Must have national certification and Commonwealth of Massachusetts licensure as a Physician Assistant
  • NP/PA – Must possess and maintain, or be eligible for, Mass DPH Controlled Substance registration and DEA registration
  • Must be able to regularly travel to multiple locations, to patients’ homes, which may or may not be accessible by public transportation
  • Minimum two years of APC experience in ambulatory primary care to include chronic disease management

Preferred Qualifications:

  • Geriatric experience
  • Homecare experience
  • Fluency in EMR systems and technology
